We have an opening for an LPN. Candidates must be self motivated and service oriented.
Experience working in an assisted living/memory care environment is a preferred. Must have strong leadership and communication skills.
Schedule: This is a full time position and includes week days as well as every other weekend rotation.
Assisted Living - ADMINISTRATIVE DUTIES
- Perform administrative duties such as completing medical forms, reports, evaluations, and charting as needed.
- Complete Accident/Incident reports as needed. Notifies resident’s family, Physician, and DON, documents in resident’s chart.
- Update resident electronic record, coordinate with Medical Asst.
- Communicate changes regarding residents and employees daily to DON.
Assisted Living / PERSONNEL DUTIES
- Assist DON in directing the day-to-day functions of the PCAs/ Med Passers in accordance with current policies and procedures and the regulations and guidelines that govern a licensed Home for the Aged.
- Assist PCAs as needed in developing work assignments, and/or assist in completing and performing such tasks.
- Make routine rounds to ensure work is being performed as assigned. Report problems to the DON.
- Participates in orientation program that orients new employee to the unit’s policies and procedures, and to his/her job position and duties.
Assisted Living/ Memory Care- RESIDENT CARE SERVICES
- The LPN ensures residents right to privacy and maintain dignity and respect of residents.
- Cooperate with other resident services when coordinating Resident Health Services to ensure that the resident’s overall care is maintained.
- Assist in admitting, transferring, and discharging of residents as required. Help new residents and families orient to unit.
- Encourages attending physician to record and sign their orders, progress notes etc.
- Document routinely in resident’s medical record in an informative and descriptive manner.
- Receive written/verbal orders from physicians and record on Physician’s Order Form, resident’s chart, and medication administration records as required. Order prescribed medication per physician order and administer as ordered.
- Report all discrepancies noted concerning physicians orders, charting errors, etc. to DON.
- Reviews medication administration record (MAR) for completeness of information, accuracy in the transcription of the physician's order, and adherence to established policies.
- Notifies DON of all drug and narcotic discrepancies. Complete medication incident report as needed.
- Participates in resident care reporting.
- Assists and respond to family needs.
- Collaborates with DON in making decisions concerning resident care.
- Perform daily rounds to assure that prescribed treatments are being properly administered by RCA's and to evaluate the resident’s physical and emotional status. Monitor health care service areas ensuring they are maintained in a clean and safe manner.
- Requisitions and arranges for diagnostic and therapeutic services as ordered by the physician and in accordance with established procedures. Report results to physician.
- Obtains specimens, take and record TPRs, (temperature, pulse, respiration) blood pressure, etc. as needed
- Reviews Level of Care routinely with RCA’s to ensure that appropriate care is being rendered. Update service plans as needed.
- Informs the DON and Director of Resident Health of any changes that need to be made on the Level of Care plan.
Qualifications
GENERAL REQUIREMENTS:
Must be able to execute all terms and conditions set forth in the JSL Employee Handbook, including but not limited to:
- LPN complies with company policies and procedures and local, state, and federal regulations.
Licensed Practical Nurse (LPN) PHYSICAL DEMANDS:
Lifting of approximately 25 pounds with occasional lifting of residents and small objects, sitting, walking, talking and hearing.
