GENERAL JOB DESCRIPTION |
| The Intake Case Manager is responsible for recruiting individuals with disabilities for Community Inclusion (Skill building/Supported Employment) services in Wayne and Oakland Counties. This role includes conducting outreach to schools, funders, and referral sources, as well as conducting recruitment events. The Intake Case Manager is accountable for meeting growth targets, coordinating and documenting tours and intake meetings, and completing all required intake documentation. The Intake Case Manager will maintain a caseload of new referrals, develop initial service plans, and train staff on participant goals and needs. This position requires strong communication and time-management skills to maintain high standards of responsiveness, customer service, quality compliance, and continuity of care. QUALIFICATIONS | | Education: Bachelor's degree in psychology, social work, special education or related field required. Other: - 21+ years of age.
- Criminal background must meet funding source requirements.
- Reliable transportation and valid Michigan driver’s license.
- Driving record must be approved by Agency insurance carrier.
- Complete all assigned trainings including but not limited to CPR, First Aid, and Recipient Rights.
- Two years related work experience serving individuals with disabilities required.
- Strong interpersonal skills to communicate clearly and professionally with large groups, funders, families, participants and staff.
- Composition skills sufficient to prepare required documentation.
- Fluency in American Sign Language skills is a plus.
- Basic computer skills.

DUTIES AND RESPONSIBILITIES |
- Cultivate referral relationships with schools, funders, and community partners, and plan targeted recruitment events to meet annual enrollment growth projections.
- Respond to all referral inquiries promptly and oversee tours and intake meetings.
- Complete intake packets for new referrals, develop plans, and provide interim case management support.
- Train staff on each new participant’s Individual Plan of Service (IPOS) and document training accordingly.
- Collaborate with program leadership and other staff to ensure appropriate and expeditious service placement and financial resources for benefits planning.
- Provide applicable referral paperwork to Supported Employment and/or Skill Building staff to help assess the participant’s vocational skills, needs, and interests.
- Works closely with Gesher’s Customer Service Representative to ensure compliance with policies.
- In partnership with the participants and staff, develop and implement services and supports to assist participants with learning and maintaining skills that maximize and promote a healthy, active and socially integrated lifestyle.
- Using a person-centered approach, provide case coordination, follow-along supports, linkages and advocacy services to participants.
- Develop, monitor, and periodically review support plans for designated participants.
- Secure and maintain required authorizations for service.
- Prepare and submit necessary documentation in computerized databases for funding source in a timely manner.
- Generate reports and other documents to ensure that the case record is in accordance with all applicable regulations for governing accreditation and funding bodies.
- Establish and maintain positive and outcome-oriented relationships with participants and their involved others, community partners, other team members, and funding sources.
- Transport participants using Agency vehicle or own vehicle, as needed.
- Perform other duties as assigned.
- Serve on Agency committee(s) as appointed.

WORKING CONDITIONS |
| Environmental conditions: - Moderate noise (i.e., business office with computers, phone, and printers, light traffic).
- Ability to work in a confined area.
- Ability to sit at a computer terminal for an extended period.
- Available to provide evening or weekend coverage as needed
Physical requirements: - While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone keyboard, reach, stoop, kneel to install computer equipment.
- Specific vision abilities required by this job include close vision requirements due to computer work.
- Light to moderate lifting in required.
