JOB TITLE: Supports Services Coordinator

DEPARTMENT: Senior Adult Services

SUPERVISOR: Manager, Senior Adult Services

GENERAL JOB DESCRIPTION

To create, promote, and implement engagement opportunities for adults 55 and older while providing supports and referrals as needed with aging and cognitive changes.

QUALIFICATIONS

  • Master’s degree in social work, guidance and counselling, psychology, gerontology, or related field preferred.
  • Two to three years of experience working with seniors.
  • Interpersonal skills and cultural sensitivity sufficient to effectively relate to and communicate with employers, participants, funding sources, public and staff.
  • Composition skills sufficient to prepare required reports, grants, correspondence, etc.
  • Driving record must meet safe driving standards as established by Agency insurance carrier.
  • Valid Michigan driver license.
  • Ability to work flexible hours and travel to local community sites.

DUTIES AND RESPONSIBILITIES

  • Develop and implement programs for adults 55 and older as well as family care partners served by the Senior Adult Services Department.
  • Assess the ongoing capabilities of program participants to engage meaningfully in diverse programming, and adapt the programming and/or obtain information about community resources needed for referral.
  • Work with the Director, Senior Adult Services to develop and implement programs for participants.
  • Work with the Director, Senior Adult Services to develop appropriate volunteer opportunities within the community to match the interests and abilities of program participants and, as necessary, instruct participants in the completion of work, perform quality inspections of work completed, and serve as liaison to organizations in the community providing the work.
  • Assist with marketing the program in the community to ensure sufficient and appropriate opportunities for participants and to increase program participation.
  • Work with the Director, Senior Adult Services in planning and coordinating social and/or cultural events, including participant appreciation events.
  • Work with the Director, Senior Adult Services to recruit, screen, select, and orient program participants.
  • Maintain case records and complete other mandatory documentation, maintaining confidentiality of program participants and family care partners.
  • Track outcomes and complete, submit and maintain reports as required.
  • Serve on agency committees as appointed.

WORKING CONDITIONS

Environmental conditions:

  • Moderate noise (i.e., business office with computers, phone, and printers, light traffic).
  • Ability to work in a confined area.
  • Ability to sit at a computer terminal for an extended period.

Physical requirements:

  • While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone keyboard, reach, stoop, kneel to install computer equipment.
  • Specific vision abilities required by this job include close vision requirements due to computer work.
  • Light to moderate lifting in required.
